Welcome to help documentation for the Community Care Coordination (CCC) component of the Unified Care Continuum Platform (UCCP)!
The CCC is a comprehensive case management and tracking tool that was designed for use by care coordinators, behavioral health, medical providers, case managers, as well as leaders across the Care Continuum (program directors, CMOs, policymakers, and more). This manual explains the purpose and function of the many features within the CCC.
